Document Exchange

The residence permit shall be exchanged in the case of:

• changes to the data contained in the document,

• damage to the document,

• loss of the document,

• changes in the facial image of the holder of the document in relation to the facial image placed in the document to an extent that makes it difficult or impossible to identify the holder.

• the Republic of Poland taking over responsibility for the international protection of the holder of a residence permit issued in connection with the granting of a residence permit for a long-term EU resident with an annotation about the Member State of the European Union that granted international protection and the date of its granting;

• the assumption by another Member State of the European Union of responsibility for the international protection of the holder of a residence card issued in connection with the granting of a long-term EU resident's residence permit.
