Loss or damage

The holder of the document is obliged to notify the authority that issued the document to them of its damage or loss, within 3 days from the date of its loss or damage. Notification of loss or damage to the document should be made on the form, which you can download HERE.

Notification of loss or damage to a document results in the issuance by the authority of a certificate confirming this fact free of charge, which remains valid until the document is replaced, but not longer than for 2 months.

To obtain a new document due to its loss or damage, you must submit an application for its replacement.

In the case of recovery of a lost document, the authority that issued it must be notified within 3 days from the date of its recovery.

In addition, in the event of recovery of a lost document after receipt of the said document in place of the lost one, the holder of the document is obliged to return the recovered document immediately to the authority that issued it.
