Completing the application

An application for access to information on the inclusion of a foreigner's data in the list or the Schengen Information System for the purposes of refusal of entry / rectification of data / deletion of data is submitted on a form in accordance with the template set out in the Regulation of the Minister of Interior and Administration of 5 April 2023 on the template of the application form for access to a foreigner of information included in the list of foreigners whose stay on the territory of the Republic of Poland is undesirable, or the Schengen Information System for the purposes of refusing entry and stay, on the duration of the entry and the legal and factual basis for the entry, on the rectification or deletion of the foreigner's data included in this list or System (Journal of Laws item 655) containing:

• precise definition of the request*

• foreigner's data: name, surname, date of birth*

• previous name(s) and surname(s) of the foreigner

• Family name

• parents' names and mother's family names

• the series and number of the foreigner's travel document

• place and country of birth

• sex*

• nationality*

• address of residence*

• address for correspondence of a foreigner on the territory of the Republic of Poland (if different from the address of residence) or the address of a representative (if established)*

• justification of the request required* only in the case of requests for rectification of data in the Schengen list / in the Schengen Information System or deletion of data from the list / from the Schengen Information System

• handwritten signature of a foreigner or a representative authorized to sign it, or a legal representative of a minor (for applications concerning a minor foreigner).*
