Incorrect recording of personal data

When completing the application, special attention should be paid to the compliance of the foreigner's personal data with their record contained in the travel document presented.

The transcription of surnames appropriate for the Polish language in travel documents issued by the authorities of the foreigner's country of origin sometimes differs significantly from the entries contained in Polish civil status records (e.g. Markiewicz -> Markevich, Pszczyński -> Pshchynskyi). However, regardless of the judgment regarding the correctness of the record, the foreigner is obliged to indicate the data that the issuer of the travel document has included in it, or they should request the authorities of the country of origin to rectify the record of these data.

In the event that the foreigner has a name or surname in several parts, all the parts should be included in the application, in accordance with their order. The same should be done in the case of providing data of parents or other family members (data of other family members are given if they live in the territory of the Republic of Poland).

If the foreigner had other personal data in the past (name(s) or surname/surnames), they should be provided or indicated that there are no changes in this respect (by entering in the appropriate box: none). If the foreigner used other personal data, the authority should be informed about this fact.
