At the stage of completing the application

Remember!

• The application should be completed on the form, in full, legibly, in Polish. Appendix No. 1 to the application in the case of temporary residence permits for the purposes of performing work is completed by the entity entrusting the performance of work, indicating its own data, not the data of the foreigner.

• The application may concern only one person – in the case of a family, the application is submitted for each person separately.

• The application must contain information and data consistent with the facts.

• The record of personal data should be consistent with the data record in the foreigner's travel document. This note also applies to the record of the data of parents and other family members indicated in the application.

• If the foreigner has provided other personal data in the past – they should provide such information.

• When writing geographical names, equivalents of names specific to the Polish language should be used, e.g. not Kiev – but Kijów, not Paris – but Paryż.

• The application must specify the place of actual residence in Poland

• The section on criminality should indicate information on judgments handed down that have not been erased, and in the section on ongoing criminal proceedings or proceedings in cases of misdemeanours, information on all pending proceedings (e.g. on unadjusted criminal fines) should be provided.

• The application should correctly indicate the main purpose of the declared stay, and this purpose should, as a rule, justify in the case of a temporary residence permit for a stay longer than 3 months.

• The list of annexes to the application should correspond to their actual attachment.

• Remember to sign the application. The signature shall be hand-written by a person authorised by law, not by a representative. The signature should be legible, in the Latin alphabet and contain the applicant's full name(s).
