Completing the application

• The possibility of completing the application in MOS is available only to logged in users.

• When completing and submitting the application, be sure to follow the instructions given to you in the instruction to the application.

• First, go to the "My Account" tab to register (if you do not already have an account) or log in to the system (if you are not already logged in).

  In case of technical problems with registration or login, go to the "Avoid errors" section ("Database of questions and answers" tab) or – in the event that you do not find the information you need in this section – contact technical support (contact details of technical support can be found in MOS in the "Contact" tab posted on the main page of the portal).

• If you remain logged in, go to the application completion section.

• Before proceeding with the online application procedure, it is recommended to read the information about the most common errors made by applicants available in the section: "Avoid mistakes" (Click to go to the "Most common mistakes" tab).

• The application is a contextual help useful when completing the application. If you have questions about the application, go to the tab: Database of questions and answers. In case of further doubts, please contact the authority to which the application is submitted with a request to clarify them (via the contact form) or signal doubts tothe person accepting the application in the office serving the voivode.

Remember!

Completing and approving an online application does not imply its actual submission. A correctly completed application is complete only when it is hand-signed by the person authorized to submit it.

Once you have completed and approved your online application (in MOS), print out this application and then sign it. Present the application prepared in this way together with the attachments to the voivode competent for the place of your stay in Poland during the discussed visit to the office.
