Cancellation

A document shall be cancelled by the competent authority (which issued it or which exchanges it) in the case of:

• when it has been lost or damaged;

• when there is a need to replace it due to:

- changes in the facial image of the holder of the document in relation to the facial image placed in the document to an extent that makes it difficult or impossible to identify the holder,

- changes to the data contained;

• acquisition of Polish citizenship by its holder;

• the death of its holder;

• where it has not been returned after the decision to cancel it has been issued.
