Documents

  • Applicants, depending on the circumstances, should provide documents confirming the circumstances indicated in the application: e.g., confirmation of payment of the fine imposed by a court judgment, documents confirming personal data (e.g., when rectifying data, if they are incorrect, outdated).

The spouse of a citizen of the Republic of Poland applying for the removal of data from the list of foreigners whose stay on the territory of the Republic of Poland is undesirable should attach to the application:

in the case of marriage abroad: marriage certificate located in the Polish Civil Registry Office or

in the case of marriage in Poland: a copy of the marriage certificate or an officially confirmed copy of this act.

The marriage certificate may be returned to the party at the end of the administrative procedure, after prior marking of the request in the application.

Remember!

  • All documents should be drawn up in Polish or translated by a sworn translator and should be submitted in the original or, in the case of copies, should contain an official confirmation for compliance with the original.

  • Certification of conformity with the original may be made by a notary public or by the party's representative acting in the case, who is a lawyer or legal adviser.

  • If a copy of the document has been made in the form of an electronic document, the certification of its conformity with the original shall be made using a qualified electronic signature, a trusted signature, or a personal signature. Copies of documents certified electronically are prepared in data formats specified in the regulations issued on the basis of Article 18 point 1 of the Act of 17 February 2005 on computerization of the activities of entities performing public tasks.

Documents to be attached to the application - summary:

  • Power of attorney (if granted) and confirmation of stamp duty on the power of attorney,

  • Marriage certificate (in the case of spouses of Polish citizens),

  • Other documents confirming the circumstances indicated in the application.
