Document Exchange
The exchange of a Polish foreigner's identity document takes place in the case of:
changes to the data contained in the document,
damage to the document,
loss of the document,
changes in the facial image of the holder of the document in relation to the facial image placed in the document to an extent that makes it difficult or impossible to identify the holder.
Polish the foreigner's identity document is mentioned by the voivode competent for the place of stay of the foreigner.
An application for the exchange of a Polish foreigner's identity document should be submitted within 14 days after the occurrence of any of the above-mentioned conditions for its exchange, on the form that you can download HERE.
