Completing the application

  • The option to fill in the application in MOS is available only to logged in users.

  • When filling in and submitting the application, be sure to follow the instructions provided to you in the application instructions.

  • First, go to the "My account" tab to register (if you do not have an account yet) or log in to the system (if you are not logged in yet).

    In case of technical problems with registration or logging in, go to the "Avoid mistakes" section ("Question and answer database" tab) or - if you do not find the information you need in this section - contact technical support (technical support contact details can be found in MOS in the "Contact" tab on the home page of the portal).

  • If you remain logged in, go to the application completion section. Select "Temporary residence permit" if you intend to apply for a temporary residence permit for the purpose of family reunification.

  • Before proceeding with completing the online application, it is recommended to read the information about the most common mistakes made by applicants, available in the section: "Avoid mistakes" (Click here to go to the "Most common mistakes" tab).

  • The application is provided with contextual help useful in completing the application. If you have any questions about the application, go to: Question and answer database. In case of further doubts, contact the authority to which the application is submitted with a request to clarify them (via the contact form) or signal the doubts to the person receiving the application in the office servicing the voivode.

Remember!

  • Online application completion and approval do not constitute actual submission. A correctly completed application is complete only when it is signed by hand by a person authorized to submit it.

    After completing and submitting the application online (at MOS), print this application and then sign it. Present the prepared application and attachments to the voivode competent for your place of stay in Poland during the visit to the office.

  • (In the case of a minor staying in Poland – by a parent, in the case of a minor staying outside Poland – a parent or stepfather/stepmother, to whom he/she joins in Poland. The stepfather/stepmother must have the written consent of the child's parent to submit the application. Giving consent is tantamount to granting a power of attorney to act on behalf of a minor in a given proceeding.)
